Compliance documents

Many thanks for submitting your application form.  One of our lovely consultants will be in touch shortly.

In the meantime, below is a list of documents required to complete your registration.

Please note that we do require all of the mandatory documents.
Mandatory Personal Documents & Professional Qualification Details
  • Right To Work i.e. valid passport / visa
  • CCT / JCPTGP or equivalent
  • Immunisation record including Hep B, MMR, Tetanus & TB
  • CV
  • Medical Indemnity including breakdown of coverage (please ensure the certificate includes the term ‘locum’)
  • Enhanced DBS dated within three years, with all five sections checked
  • Smartcard ID number

Mandatory Training
  • Safeguarding Of Children & Adolescents Level 3 minimum completed within one year (online) or three years (face to face)
  • Safeguarding Of Vulnerable Adults Level 3 minimum completed within one year (online) or three years (face to face)
  • Resuscitation Adults & Paediatric completed within one year
Please note that for some sessions (including Out Of Hours and Home Triage), additional compliance is required.  Our consultants will let you know if anything else in particular is needed.
